Year-End Giving Statement Template

A free, copy-and-paste template for church and nonprofit year-end donation statements, plus a printable example. Use it as-is, or let the free tool fill it in for every donor from your spreadsheet.

What to include

• Your organization's name (and address; EIN is optional but common)
• The donor's name and mailing address
• The statement period, for example January 1 to December 31, 2025
• Each gift's date and amount (or totals by fund), plus the year's total
• A statement about goods or services, if none were provided, such as "No goods or services were provided in exchange for these contributions."
• A thank-you and a signature

Churches: if the only thing donors received was worship or other religious services, a common line is "No goods or services were provided in exchange for these contributions other than intangible religious benefits." Pub. 1771 describes this "intangible religious benefits" statement.

Nonprofit year-end donation statement

For a nonprofit that isn't a church, use the same template and pick the first goods-and-services line ("No goods or services were provided..."). If a donor received something in return, such as a dinner ticket, describe it and give a good-faith estimate of its value instead (Pub. 1771 explains this).

Tips

• Send by January 31. Pub. 1771 notes that charities typically send acknowledgments by January 31 of the following year.
• Double-check totals against your bank deposits before sending.
• Email or paper both work. The IRS accepts acknowledgments by email or on paper.
• No Social Security numbers. Pub. 1771 says they aren't needed on the acknowledgment.
